FEU & DEPCOMM FORMS
The Online Entry Request Form is the official form honored by the Logistics Services of FEU. This covers all entry requests for guests, food, props, and equipment. Make sure you provide all the details:
Guest(s) - complete name and plate number of vehicle if parking space will be requested
Items - (Non-electrical items like props) - dimensions, quantity, and other details
Items - (Electrical items like lights and sound equipment) - quantity and wattage
The form should be emailed to [email protected] and copy furnish the Department Chair ([email protected]) and your faculty adviser.

The Waiver is editable and can be used in ANY course (subject) that requires off-campus tours and activities. This should indicate the specific course and the objective(s) of the off-campus activity and individual expense involved. Follow these steps:
1. Download the form and edit it indicating the course, the objective and/or purpose of the activity, and the complete name of
the faculty.
2. Print the form and submit it to the Department of Communication. This will undergo vetting at the IAS Dean's Office.
3. Make a follow-up of this waiver three (3) days after submission to DepComm.
4. Produce individual copies of the waiver based on the number of student-participants of the activity.
5. Return the accomplished waiver to your faculty BEFORE the scheduled activity date.

The availability of the room, studios, and/or equipment must be determined first by any of the Lab Assistants stationed at AB 212. The accomplished form of this request MUST be endorsed by the faculty or org adviser before submission to DepComm.
